Conditions of Enrollment for ALL attenders: Every precaution is taken for the safety and good health of those attending. In the event of accident or sickness, Emmanuel Baptist Bloomfield including the Staff and Volunteers are hereby released from any liability. If a child is not cooperating with the expectations put in place, we reserve the right to contact a parent or guardian to ask the child be removed from program. In the event of an accident or sickness the parent or guardian will be notified immediately. I give permission for my child to participate in all activities and receive medical treatment if necessary. I will send my child with the appropriate equipment that they need each day such as a water bottle and running shoes.
